This form must be completed by the adult student or parent on behalf of their child. If the parent attends the lesson, the parent must also complete a separate form. Please answer these quick questions before attending the lesson, to help everyone keep safe and healthy. If you, or members of your household are feeling ill, please cancel your lesson immediately. If the student is feeling well enough, the lesson can be taken online. Please contact your teacher to arrange for the lesson to be online.

  • Symptoms of COVID-19

    Symptoms of COVID-19 can vary from person to person. Symptoms may also vary in different age groups. Children have been more commonly reported to have abdominal symptoms, and skin changes or rashes. If you have 2 or more symptoms, cancel your lesson immediately and stay home. Some of the more commonly reported symptoms include
    • new or worsening cough
    • shortness of breath or difficulty breathing
    • temperature equal to or over 38°C
    • feeling feverish
    • chills
    • fatigue or weakness
    • muscle or body aches
    • new loss of smell or taste
    • headache
    • gastrointestinal symptoms (abdominal pain, diarrhea, vomiting)
    • feeling very unwell

  • Before attending an in-person lesson, you must confirm the following:

    I, nor members of my household, have not experienced any of the symptoms of COVID-19 within the last 14 days.
    I, nor members of my household, have not travelled internationally or visited an area highly-impacted by COVID-19 in the last 14 days.
    I, nor members of my household, have not been diagnosed with or tested positive with COVID-19 or have had any contact with any confirmed case.
